In a June 3rd interview with Health Affairs, CMS senior leader Liz Fowler stated the agency’s intent to shift to mandatory value-based payment models. Fowler was recently appointed as the head of the Center for Medicare and Medicaid Innovation (CMMI), which develops and tests new ways to improve care though innovative payment arrangements. She is tasked with implementing key elements of the Biden administrations health policy agenda.
In the interview, Fowler noted that voluntary value-based care models are subject to risk selection. Providers who fare poorly in voluntary models tend to exit those programs, which limits program impact on cost and quality. Mandatory models resolve this issue, though they raise other concerns and challenges.
